Water Main Tie-in Permit Application Form Criteria

**Note: This application is required for all water main tie-ins that require modification or shut down to the adjacent water distribution system.

Contact information for a representative who will respond to public inquiries

The Applicant will be responsible for answering public inquiries regarding the water service interruption / water main tie-in work. List a designated person who would be responsible for fielding public inquiries along with their contact information. City of Port Moody clerical staff will not be responsible for handling these calls and will need to direct all public inquiries received to the City onto the Applicant’s designated contact person. This contact information will also be required on the notification shut down forms delivered to affected addresses.

**This application must be made a minimum of 15 business days prior to water main tie-in work taking place.  This allows for preparatory work in advance of modifications to the water distribution system and to provide ample time for notification of affected customers.

**Note: Please allow City staff to review and respond within 96 hours following the submission of your Water Main Tie-in Permit Application.
